• 「ことになっている」 = according to the custom/rule, ... 「この会社では8時に仕事を終えることになっている」.
• The decision-maker is hidden — this is the key point: the speaker is only conveying the rule, not taking responsibility for it.
• Also used for a finalized schedule: 「明日、社長と会うことになっている」 (already arranged to meet the director).

  One's own habit is 「ことにしている」; 「ことになっている」 is an external rule.

• ผิด会議(かいぎ)は3時(じ)に始(はじ)まることになった。

  The meeting is scheduled to start at 3 o'clock.

  ถูก会議(かいぎ)は3時(じ)に始(はじ)まることになっている。

  「ことになった」 only announces a decision that was just made; to state a schedule/rule currently in effect, use 「ている」.
